Accessing Primary Health Care

The Primary Health Care Team consists of Medical Doctors, Nurse Practitioners, Registered Nurses and Registered Practical Nurses who address acute and chronic health needs and also provide preventive, rehabilitative and palliative care.

Primary health care is offered to clients who are registered as primary care clients with the Centre.

How do I book an appointment?

Call the Centre to book appointments.

  • Open Monday to Friday 8:30 a.m. to 4:30 p.m.
  • The OCCHC doesn’t take calls from 11:30 a.m. to 1:00 p.m.
  • Open Wednesday Evening 5:30-8:30pm
  • Phone: 519-539-1111
  • Toll Free: 877-522-1112

If you are a registered primary care client and are not sure if you should come into the Centre, our nursing staff will give you guidance and help you book an appointment, if needed.

Please also book an appointment for non-urgent needs including prescription renewals at least 12 days ahead to ensure that you always have an adequate supply.

Home visits may be available for clients who have special circumstances such as palliative care needs.

Emergency Situations

In emergencies CALL 911 or the Reach Out 1-866-933-2023 immediately or go directly to the nearest hospital emergency department.

We are not equipped to deal with emergency or crisis health needs.

Early detection of illness is important. Screening for health concerns at certain times over your lifespan can help keep you healthy. Even if you feel well, it is important to take charge of your health.

We encourage you to have a complete health review every two years.

Those who have chronic illness should book regular appointments. Your provider will help determine a schedule.
