Healthy Development Supports for Children 0-5
First Five – a program for children aged 0-5 years old who do not have current access to primary care, and/or do not have an Ontario Health Card.
Eligibility:
- The child is less than 6 years old
- No primary care provider (Physician, or Nurse Practitioner)
- No Health Card required
We Do Not Provide:
- Care for children 6 years of age and older
- Care for parents or adult family members
- Obstetrical/Prenatal Care
- Emergent/Acute Illness Care
Services:
The program is here to assist these families and their children with:
- Well baby/child assessments
- Maintaining their child’s vaccination schedule
- Breastfeeding support with a lactation consultant
- Developmental screening for physical and mental health
- Assistance with navigating the healthcare system and community services
- Dietary support
