Chronic Diseases cover a range of conditions that people live with every day, and will continue to live with throughout their lives. They may impact your life in a variety of ways and requires both self-management and medical management from your health care team.
Led by our Chronic Disease Nurse Practitioner, our Chronic Disease Management Programs focus on supporting people where they are at with their chronic diseases and providing education and support for ongoing self-management.
Conditions that we support our patients with include:
- Type 2 Diabetes
- Chronic Obstructive Pulmonary Disease (COPD)
- Hypertension or High Blood Pressure
- High Cholesterol
- Asthma
- Weight Management
Living with a chronic disease can cause different feelings. Some people describe feeling overwhelmed and lonely. People who are registered with one of our Family Physicians or Nurse Practitioners can schedule a visit with our Chronic Disease Nurse Practitioner for review of medications, education support and routine follow up. We also offer group programming to support members of our local community and help people meet others with similar stories and interests.
